The short answer. Medical Guardian is a strong all-round choice, particularly if needs might change. Its four devices — two home bases, a mobile device and a smartwatch — cover someone who is mostly home through to someone who is out every day. Medical Guardian states no long-term contract and no equipment, activation or cancellation fees, 24/7 U.S.-based monitoring, and a home range of up to 1,400 feet. We have not tested it hands-on; everything below comes from Medical Guardian's own published information.
The four devices, and what each one is for
Medical Guardian's current line-up is four devices.[1]
- MGHome Landline and MGHome Cellular. In-home base stations, over a phone line or on the cellular network. Medical Guardian states a home range of up to 1,400 feet for its at-home devices, which it describes as the longest in the industry.[3]
- Guardian Flex. The mobile device, with nationwide GPS. Medical Guardian states it can stay charged for up to five days and is water resistant.[1]
- MGMove. A smartwatch with step tracking and weather. Medical Guardian states a battery life of up to 24 hours — so it needs charging daily — and an IP67 rating: fine in the shower, but not for submerging.[4]
Because all four sit on the same service, a change in someone's routine means changing the device rather than changing company.
What family can see
Through Medical Guardian's portal and app, family can view the MGMove's location history, which Medical Guardian says is updated hourly, see its real-time location when the SOS button is pressed, and follow step data.[4]
Fall detection
Fall detection is an available add-on rather than part of the standard service, including on the MGMove.[1][4] Ask whether it is included for the device and plan you are choosing.
Fees and contract
Medical Guardian's FAQ states that it does not require a long-term contract and does not charge equipment, activation or cancellation fees.[2] Its home page footer, however, says that "cancellation fees may apply". The two statements do not match, so ask for the cancellation terms in writing before you order.
Who it suits
Someone whose needs may change over the next year or two, or a family that wants to see location and activity. The range of devices on one service is the main reason to choose it.
Who should look elsewhere
Someone who needs a smartwatch but will not charge it every day: the MGMove's stated battery is up to 24 hours. And anyone who wants cancellation terms settled before ordering, until Medical Guardian's own pages agree.

How it compares on our criteria
Against our criteria, Medical Guardian's strongest points are equipment and coverage — four devices, a 1,400-foot stated home range — and fees, where its FAQ states none for equipment, activation or cancellation. The contradiction with its own footer on cancellation fees is the thing to resolve first. We cannot assess monitoring response ourselves, because we have not tested it.
